Activate your Gator 1 Card

This form is for Health Science Center students, staff, and faculty only. All other University of Florida patrons should use the service provided by the Smathers library at: http://web.uflib.ufl.edu/distreg.html

Shands and VA personnel must complete a courtesy borrower application at the circulation desk to obtain privileges.

Activating your Gator1 Card allows you to check out library materials in the library and gain access to remote databases. Please complete all requested information in the form below. Incomplete forms cannot be processed. Allow one business day from receipt for processing. You will receive an e-mail confirmation when your card has been activated.
